Reports

After completion of the assessment, the ASAM CONTINUUM will generate two comprehensive reports: the Summary and Narrative Reports.

Summary Report

The ASAM CONTINUUM Summary Report contains biopsychosocial diagnostic information regarding the patient's substance use, co-occurring considerations, and the final level of care recommendation.

This includes:

  • DSM-5-TR SUD diagnoses
  • ASI composite scores
  • CIWA and CINA withdrawal scores and presence of imminent withdrawal risk

Narrative Report

  • Relevant biopsychosocial history
  • Patient and provider perception of problems and need for treatment
  • ASI Composite scores
